A COMPLETED APPLICATION consists of the following:

__ Application Fee: $100.00 Non-Refundable

__ Home Language Survey

__ Authorization for Emergency Care

__ Student Pickup Authorization Form

__ Student Media and Directory Release Form

__ Student Health Form

__ Parent Questionnaire

__ Recommendations (Teacher, Principal or Pastor)

__ Report Card/Transcript (most recent)

__ Request for Confidential Records

__ Copy of Birth Certificate

__ Copy of Social Security Card

__ Copy of Child’s Immunization Record (must be up-to-date)

__ Family Photo (4x6 snapshot is acceptable)

__ Any ARD paper work including Full Individual Evaluation (FIE)

__ Current Individual Education Plan (IEP)

__ Reports from special testing (learning needs / psychological tests)

__ Any paper work for special services received

__ Any legal information that is relevant to the child’s safety and well being
